I think the final shot on the wife is intentionally ambiguous. She could’ve done it but there’s no evidence put in (to the episode) that she’d be capable of the surgery involved in the murder. Plus we know she was home! The writers are too good to not set that up. I think she intentionally testified in a way to nail him but he did do it.
